Navy Safety Command publishes a report on aviation, ashore and afloat mishaps at the end of each fiscal year that includes off duty and crash deaths, but does not report on deaths from overdoses, suicides or illnesses. The report only contains Class “A” mishaps, which means there were aircraft lost, damage of at least $2,500,000, a fatality or a permanent disability.
